Honours and Awards

Recognising the service of Australians who have made a significant contribution to our nation is important.

Many people have contributed to our great nation in a range of fields and they deserve to be properly honoured.

Military and National Service medals are administered by the Department of Defence whilst the Order of Australia is administered by the Honours Secretariat at Government House.

Below you will find links to the application and nomination forms for Defence medals and the Order of Australia. Completed forms may be returned to my office at 42 Florence St, Mentone VIC 3194 for submission.
